Infinite Medical P.C is hiring a Primary Care Nurse Practitioner/Physician's Assistant to provide on-site care to residents in long-term care settings in Chandler, OK. This part-time role (3 days per week) supports meaningful patient relationships and collaborative care with a multidisciplinary team.

You will perform exams, manage chronic and acute conditions, order tests, develop care plans, and educate residents and families.

Qualifications

• Active and unrestricted Nurse Practitioner, Physician Assistant, or Physician license in good standing.
• State-specific license in Oklahoma in good standing.
• Minimum 6 months of experience in long-term care or skilled nursing facilities.
• Strong communication, clinical and critical thinking skills.

Responsibilities

• Provide primary care services to residents in skilled nursing and long-term care facilities.
• Perform physical exams and medical history assessments.
• Diagnose and treat chronic and acute health conditions.
• Order and interpret diagnostic tests and lab results.
• Develop and adjust individualized care plans.

Requirements

• Passion for elder care and person-centered healthcare
• Prolonged standing/walking. Occasional lifting up to 20 lbs. Computer work. Lifting/moving patients. Prolonged sitting.
