Infinite Medical P.C. in Berlin, Vermont, is hiring a Primary Care NP/PA to provide on-site care in long-term care settings. This full-time role offers a meaningful patient relationship and a collaborative care environment for residents.

You will perform exams, manage chronic and acute conditions, prescribe medications, and work with interdisciplinary teams to deliver compassionate elder care. Salary ranges from $150,000 to $200,000 per year with full benefits and a signing bonus.

Qualifications

• Active and unrestricted Nurse Practitioner, Physician Assistant, or Physician license.
• State-specific license in good standing in Vermont.
• Minimum 6 months of experience in long‑term care or a skilled nursing facility.
• Must be able to lift a minimum of 20 pounds and stand/walk/work on your feet up to 6–8 hours per day.

Job Summary

Infinite Medical Group is currently hiring a Primary Care Nurse Practitioner/Physician's Assistant (NP/PA) to provide on‑site care to residents in long‑term care settings. This is a full‑time position ideal for a provider who values meaningful patient relationships and a collaborative care environment.

Responsibilities

• Provide primary care services to residents in skilled nursing and long‑term care facilities
• Perform physical exams and medical history assessments
• Diagnose and treat chronic and acute health conditions
• Order and interpret diagnostic tests and lab results
• Develop and adjust individualized care plans
• Prescribe medications and monitor treatment effectiveness
• Collaborate with facility staff and interdisciplinary teams
• Educate patients and families about treatment options and care goals
• Ensure a safe, respectful, and supportive environment for residents
• Prolonged standing/walking. Occasional lifting up to 20 lbs. Computer work. Lifting/moving patients. Prolonged sitting.

Requirements

• Active and unrestricted Nurse Practitioner, Physician Assistant, or Physician license
• State‑specific license in good standing in Vermont
• Minimum 6 months of experience in long‑term care or a skilled nursing facility
• Strong communication, clinical, and critical thinking skills
• Passion for elder care and person‑centered healthcare
• Must be able to lift a minimum of 20 pounds and stand / walk / work on your feet up to 6‑8 hours per day
