Job summary

Bloom Healthcare is seeking a Primary Care NP/PA to deliver patient-centered medical care in homes and community settings, serving medically complex seniors with autonomy and strong interdisciplinary support. This role emphasizes relationship-based care and measurable health improvements, supported by robust data systems.

You’ll provide longitudinal primary care, respond to acute needs, and coordinate care with an MD leadership and triage team, all while maintaining a manageable patient panel

Qualifications

• Master’s degree required in nursing or physician assistant.
• Active Colorado licensure.
• Experience in home- or community-based primary care preferred.

Responsibilities

• Serve as the primary care provider for a defined patient panel, delivering longitudinal, holistic medical care in patient homes and communities
• Diagnose, manage, and treat chronic and acute conditions with support from the team
• Address social determinants of health, including housing stability, nutrition, caregiver support, and access barriers
• Conduct specialty visits such as wound care and post-hospital transitional care
• Complete comprehensive evaluations including medication reconciliation and health education for patients and caregivers
• Utilize Bloom’s data ecosystem to track hospitalizations and coordinate timely post-discharge visits
• Collaborate with internal and external care teams to ensure continuity and reduce readmissions

Education

What You’ll Do

• Serve as the primary care provider for a defined patient panel, delivering longitudinal, holistic medical care in patient homes and communities
• Diagnose, manage, and treat chronic and acute conditions with support from our clinical operations, triage team, and MD leadership
• Address social determinants of health, including housing stability, nutrition, caregiver support, and access barriers
• Conduct specialty visits such as wound care, post-hospital transitional care, and condition-specific assessments
• Complete comprehensive evaluations including medication reconciliation, fall risk assessments, advance care planning, and health education for patients and caregivers
• Utilize Bloom’s integrated data ecosystem to track hospitalizations, coordinate timely post-discharge visits (within 72 hours), and drive superior outcomes
• Collaborate closely with internal and external care teams to ensure continuity, reduce readmissions, and stabilize high-risk patients

Compensation

$100,000 – $125,000 annually, depending on experience, with opportunity for performance-based bonuses and full benefits.
