Hartford HealthCare seeks a Primary Care Nurse Practitioner or Physician Assistant for our Plainville Primary Care practice in Plainville, CT. The provider will manage their own patient panel and deliver patient-centered care within a collaborative team, serving a Spanish-speaking population.

The role is 100% outpatient primary care for adults (18+), full-time on weekdays with no call. A strong, stable team environment is emphasized with support from engaged providers.

Qualifications

• Licensure: Unrestricted State of Connecticut Medical License (or eligible).
• Training: APRN (Family, Adult/Geri Nurse Practitioner) or PA.
• Experience: New graduates with strong internal medicine/primary care backgrounds are welcome to apply.
• Preferred: 2+ years’ experience in a primary care or relevant outpatient setting.

Responsibilities

• Perform routine physical examinations and develop medical, educational, and psychosocial plans to treat patients.
• Develop plans for further evaluation or referral in collaboration with physicians and care team.
• Provide appropriate treatment within scope of practice as designated by protocols.

Our centrally located Plainville Primary Care practice offers a strong, stable team dynamic and a supportive work environment, led by highly engaged providers who are passionate about delivering quality care to the community. This position is excellent for providers interested in working with a Spanish-speaking population! The Primary Care Provider will carry their own patient panel and provide quality, patient-centered care among a collaborative team.

Role Scope

Under established guidelines and in collaboration and/or supervision with the practice physician(s), Nurse Practitioners and Physician Assistants perform routine physical examinations, and formulate medical, educational, and psychosocial plans to treat patients.

In consultation with the physician(s) and care team members, Nurse Practitioners and Physician Assistants develop plans for further evaluation of health needs, e.g., referral to other health team members or specialists.

Nurse Practitioners or Physician Assistants provide appropriate treatment for problems within the scope of the provider’s practice as designated by protocols and identifies problems outside of their scope of practice and refers appropriate to physician.
