Job summary

Oak Street Health seeks a Nurse Practitioner / Advanced Practice Provider to deliver high-quality primary care to adults on Medicare in underserved communities. In this patient-centered, team-based role, you will conduct comprehensive assessments, manage chronic conditions, provide preventative care, and coordinate with physicians, nurses, and social workers.

You will educate patients and families, support care plans, and use data and EMR tools to improve outcomes.

Qualifications

• Licensed NP or APRN with eligibility to practice in the state.
• Experience delivering primary care and coordinating care teams preferred.
• Comfort working with Medicare populations in underserved communities.

Responsibilities

• Provide comprehensive primary care for adults on Medicare.
