Qualifications

• Active Nurse Practitioner license in Arizona.
• Experience in primary care settings preferred.
• Comfortable working in a multidisciplinary, patient-centered team.

Responsibilities

• Perform comprehensive assessments of adults on Medicare.
• Diagnose and manage acute and chronic conditions.
• Develop preventative care plans and coordinate services within a multidisciplinary team.
• Provide patient education and medication management.
• Order and interpret diagnostics; maintain accurate EMR documentation.
