Job summary

Infinite Medical P.C. seeks a Primary Care Nurse Practitioner to provide on-site care to residents in long-term care settings in Vermont. This full-time position emphasizes compassionate, patient-centered care within a collaborative team.

You will perform exams, manage chronic and acute conditions, coordinate with facility staff, and educate residents and families. Vermont NP licensure and active license are required.

Qualifications

• Ensure a safe, respectful, and supportive environment for residents

Requirements

• Active and unrestricted Nurse Practitioner
• State-specific license in good standing in Vermont
• Minimum 6 months of experience in long-term care or a skilled nursing facility
• Strong communication, clinical, and critical thinking skills
• Passion for elder care and person-centered healthcare
• Prolonged standing/walking. Occasional lifting up to 20 lbs. Computer work. Lifting/moving patients. Prolonged sitting.
