Infinite Medical P.C. is hiring a Primary Care Nurse Practitioner to provide on-site care to residents in long-term care facilities. The role is full-time, emphasizing meaningful patient relationships and collaborative, mission-driven care.

The candidate will perform exams, manage chronic and acute conditions, and work with a multidisciplinary team to deliver patient-centered outcomes in a skilled nursing setting.

Qualifications

• Active and unrestricted Nurse Practitioner license.
• State Vermont license in good standing.
• Minimum 6 months of LTC or skilled nursing facility experience.
• Strong communication, clinical, and critical thinking skills.
• Passion for elder care and person-centered healthcare.

Responsibilities

• Provide primary care to residents in long-term care facilities
• Perform physical exams and medical history assessments
• Diagnose and treat chronic and acute health conditions
• Order and interpret diagnostic tests and lab results
• Develop and adjust individualized care plans
• Prescribe medications and monitor treatment effectiveness
• Collaborate with facility staff and interdisciplinary teams
• Educate patients and families about treatment options and care goals
• Ensure a safe, respectful, and supportive environment for residents

• State-specific license in good standing in Vermont
• Minimum 6 months of experience in long-term care or a skilled nursing facility
• Strong communication, clinical, and critical thinking skills
• Passion for elder care and person-centered healthcare
• Prolonged standing/walking. Occasional lifting up to 20 lbs. Computer work. Lifting/moving patients. Prolonged sitting.
