Provide compassionate primary and preventive care to a diverse patient population in a community-based setting. Perform comprehensive assessments, diagnose and manage acute and chronic conditions, prescribe medications, and coordinate patient care with an integrated healthcare team that includes behavioral health and specialty providers.
