Responsibilities

• Partner with Primary Care Providers to support care coordination and follow-up for patients and families.
• Assist with treatment procedures and carry out provider orders with precision and care.
• Perform patient assessments and collect data through history-taking and physical evaluations.
• Monitor patient status and respond to clinical needs using sound nursing judgment.
• Provide education and counseling to patients and families about diagnoses, treatments, and care plans.
• Direct patient care: vitals, medication administration, and monitoring.
• Coordinate specialty care referrals and follow-ups.
• Document all interactions accurately in the EMR.
• Triage patient concerns to ensure timely, appropriate care.
• Collaborate with team members to support a positive clinic environment.
