Qualifications

• Graduation from an accredited school of nursing is required.
• Candidate must hold a current New Hampshire Nurse/LPN license.
• A minimum of two years of acute care or ambulatory care experience is required.

Responsibilities

• Perform nurse visits including INR checks, vaccines, medication administration, and EKG.
• Manage triage calls and nursing judgments on medications, referrals, immunizations, and lab results.
• Communicate abnormal results to patients and coordinate with providers and care management.
• Participate in daily team huddles and health education for preventive care.
• Maintain clear, compassionate communication with patients and families.
