Role:

The Primary Care Patient Care RN or LPN provides safe, effective, patient-centered care under the primary care operations team, collaborating with providers and the clinical team to improve quality outcomes.

Responsibilities:

Partner with patients and staff to deliver systematic care, perform clinical duties, and ensure alignment with organizational standards and values.

Qualifications:

Must be a graduate of an accredited nursing program, hold an RN or LPN license, and preferably have 6 months of primary care experience. Physical demands include lifting, standing, walking, and various movements required for clinical duties.

Certifications:

Basic Life Support (BLS) through the American Heart Association or completion within 30 days of hire; additional certifications such as ACLS, PALS, or specialized unit certifications may be required depending on the role.
