Job summary

TMS in Brattleboro, Vermont, seeks Nurse Practitioners or Physician Assistants to deliver outpatient primary care with limited post-acute coverage in SNFs and assisted living.

Openings: 2–3 providers with flexible scheduling and engagement options (locum, locum-to-perm, or permanent). Expect about 16 patients per day, focus on adult and geriatric care, Cerner/Oracle EMR, and about one call per day.

Qualifications

• Family Medicine certification/training for NP or PA practice is required.
• Adult patient care experience is required.
• Geriatric patient care experience is required.
• Primary care experience is preferred.

Responsibilities

• Provide comprehensive outpatient primary care services.
• Conduct patient evaluations and manage routine medical conditions.
• Deliver preventive care and health maintenance services.
• Diagnose and manage chronic diseases.
• Provide care for adult and geriatric patients.
• Provide post-acute care services in SNF and assisted living settings as needed.
• Document patient encounters and treatment plans using Cerner / Oracle
