Qualifications

• Program Graduate
• Advanced Nurse or Physician Assistant
• No minimum experience required
• Bilingual English/Spanish (and other local languages)
• CPR for Healthcare/BLS Prov or Prof Rescuers within 30 days
• Advanced Practice RN or Physician Assistant Lic & NCCPA Certification
• ACLS or PALS (as applicable) within 30 days of hire
• State of New Mexico and Federal DEA Certification

Responsabilités

• Adhere to and promote the core expectations of the Patient Centered Medical Home or Patient Centered Specialty Practice as applicable
• Assess, diagnose and develop treatment plans; implement treatment, follow-up and evaluate patient within the scope of practice protocols
• Evaluate and revise intervention care/treatment plan with patient and family as necessary to achieve improved health
• Provide education for patients and/or families about preventative care, medical issues and use of the prescribed medical treatment
• Document accurate documentation of patient care in the medical record and communicate to health care team
• Perform invasive and non-invasive diagnostic and therapeutic procedures
• Act as a role model through conduct, leadership and ethical decision-making
• Work as a team member with medical/nursing/counseling staff to provide comprehensive care
• Guide, teach, support and serve as a clinical resource for nursing and medical staff
• Support research by conducting or participating in studies and applying findings
• Enhance professional growth through education, reading, and attending in-services and workshops
• Follow patient safety policies and identify opportunities to improve safety
• Demonstrate proactive approach to patient safety by questioning current policies
• Identify and report environmental conditions that may pose risk to patients
• Report potential or actual patient safety concerns and errors promptly
• Encourage patients to participate in their own care by asking questions and reporting concerns
