Job summary

Greater New Bedford Community Health Center in New Bedford, MA seeks a Nurse Practitioner or Physician Assistant to provide integrated primary care to diverse and vulnerable populations. You will examine patients, review histories, interpret tests, diagnose illnesses, and develop evidence-based treatment plans in collaboration with supervising providers.

This role requires thorough documentation in the EHR, adherence to compliance, and active participation in committees and care teams.

Qualifications

• Graduate from an accredited NP/PA/NM program.
• Massachusetts license or eligibility required.
• National board certification required (AANP/ANCC/NCCPA/AMCB).
• DEA registration required.
• Buprenorphine waiver may be required for OBAT roles.
• HIV certification/experience preferred.
• Psychiatric certification preferred for PMHNP track.
• Life Support Certification preferred.

Responsibilities

• Provide direct patient care including assessment, diagnosis, treatment, and management of acute and chronic conditions.
• Perform comprehensive histories and physical examinations.
• Order, interpret, and follow up on diagnostic tests.
• Prescribe medications and treatments per state/federal regulations.
• Deliver preventive care, immunizations, and health education.
• Develop and implement patient-centered care plans.
• Collaborate with physicians, nurses, and care coordinators; make referrals as needed.
• Document encounters accurately in the EHR.
