Job summary

Qualifications

• Must hold a current license from the State of Ohio.
• BLS certification required.
• 2–3 years of clinical LPN experience preferred.

Responsibilities

• Perform initial and ongoing health status assessments and document data.
• Triage patient calls, manage clinical messages and refill requests.
• Coordinate referrals and respond to MyChart communications.
• Provide direct patient care: vitals, injections, EKGs, wellness visits.
