Job summary

Ascension Medical Group in Bartlesville, OK seeks a Licensed Practical Nurse for full-time primary care responsibilities. You’ll support patient intake, administer vaccinations and basic treatments, and help with procedures in a collaborative clinic setting.

You’ll coordinate care, follow-ups, and referrals, while delivering patient education to promote health maintenance and adherence to treatment plans in a faith-based, mission-driven environment.

Qualifications

• Current LPN licensure in OK or KS prior to hire date.
• BLS Provider certification within 1 month of hire date.
• High School diploma equivalency or 1 year of applicable experience.

Responsibilities

• Streamline patient intake and assessment to support clinic flow and diagnostic readiness.
• Safely administer medications, immunizations, injections, and wound care, and assist with minor procedures.
• Coordinate care with triage, lab results, prescriptions, and referrals to ensure follow-up.
• Provide health education and chronic disease management guidance to improve adherence.
