Job summary

Mercy is seeking a compassionate Primary Care Nurse (RN or LPN) to deliver patient-centered care in an outpatient setting. You will perform assessments, administer medications and vaccines, coordinate chronic disease management, support providers during exams, and educate patients and families on treatment plans and preventive care.

You will document accurately in the EMR, triage patient calls, and collaborate with an interdisciplinary team.

Qualifications

• Licensed RN or LPN with active credential.
• Experience delivering primary care or outpatient nursing.
• Strong communication and education skills.
• Ability to triage and coordinate chronic disease management.

Responsibilities
