Job summary

Optum Care Delivery is seeking an Advanced Practice Clinician to join Reliant Medical Group in Worcester, MA. You’ll work in a patient-centered, team-based environment with physicians, PAs, RNs, MAs and admin support, delivering value-based care across the continuum.

Qualifications

• Able to obtain Board Certification as a Physician Assistant or Family Nurse Practitioner
• Certification to care for patients of all ages- pediatrics through adults
• Able to obtain Unlimited Licensure in the state of Massachusetts
• A minimum of 2 years of experience working in primary care
• Active DEA License

Responsibilities

• Patient centered team-based environment
• Will join as a AP Practice Partner working with a patient centered team
• Team inclues: Physician, AP, RN, MA, and a PSS (admin)
• Provide Care across the continuum, focusing on value-based care and suported by a robust team of specialist
• Behavioral health providers are integrated into the primary care teams
• Fewer patients per day, longer patient visits
