Job summary

American Family Care, Inc. seeks an Advanced Practice Provider to deliver high-quality care in urgent and primary care settings. You will work with a team to manage patient flow, perform histories, exams, orders, and referrals in a walk-in environment.

Active state licensure, board certification (ANCC/AANP/NCCPA), DOT, and ER/UC experience are preferred. AFC offers a flexible schedule and comprehensive benefits package including health, vision and dental insurance, retirement plan, CME, and RVU

Qualifications

• Board-certified as PA or NP with current licensure.
• Active license in the state of practice and DEA registration.
• DOT certification required.
• Ability to multi-task and work independently.
• Primary/urgent care experience preferred.

Responsibilities

• Take comprehensive histories and perform physical exams.
• Order labs, imaging, and other diagnostic studies as needed.
• Form diagnoses and arrange appropriate referrals or treatments.
• Provide patient and family health guidance and education.
• Manage patient flow and respond to emergencies as required.

Job Summary

The Advanced Practice Provider (APP) cares for our patients within the scope of training and approved the agreement, as outlined by their governing board. Works collaboratively with other providers and staff to ensure efficient patient flow and a high level of patient satisfaction.

Responsibilities

• Perform complete, detailed, and accurate health histories, review patient records, develop comprehensive medical assessments, and order laboratory, radiological and diagnostic studies appropriate for complaint, race, sex, and physical condition of the patient
• Formulate medical and nursing diagnoses and institute therapy or referrals of patients to the appropriate health care facilities, agencies, and other resources of the community or physician
• Institute emergency measures and emergency treatment or appropriate stabilization measures in situations such as cardiac arrest, shock, hemorrhage, convulsions, poisoning, and allergic reactions
• Interpret and analyze patient data to determine patient status, patient management and treatment
• Provide instructions and guidance regarding health care and health care promotion to patients/family/ significant others
• Other duties and responsibilities as assigned

Qualifications

• ANCC, AANP, or NCCPA Board Certified – (with 1 yr ER/UC experience preferred outside of clinical rotations)
• Active and current Physician Assistant or Nurse Practitioner licensure in the state of practice, DEA number, and state-controlled substance certificate, as required
• Able to multi-task and work independently
• DOT Certification required
• Primary Care and Urgent Care Experience Preferred
