Job summary

Sixteenth Street Community Health Centers in Milwaukee seeks a dedicated Prevention Program staff member focused on outreach and HIV prevention. You will coordinate outreach activities, promote HIV-related information, and connect clients to Counseling, Testing and Referrals (CTR) services during office visits or community events.

Ideal candidates hold a health/social services degree, are fluent in English and Spanish, and possess strong organizational skills.

Qualifications

  • Bachelor's or Associates degree in a health or social service-related field.
  • Excellent oral and written communication skills. Bilingual (Spanish/English) fluency strongly preferred.
  • Excellent organizational skills and ability to work independently and in a group setting.
  • Proficiency with technology such as MS Office, Electronic Health Records and other internet-based applications.
  • Valid Wisconsin driver's license, use of an automobile and current automobile insurance.

Responsibilities

  • Coordinate and conduct outreach activities aligned with HIV Prevention and Harm Reduction best practices and community needs.
  • Promote Ryan White and HIV-related information, events, and updates online or in promotion materials.
  • Identify and conduct testing for individuals at high risk for HIV infection and facilitate access to CTR services.
  • Provide CTR services during office visits or outreach activities; outreach may occur evenings or weekends.
  • Perform Social Determinants of Health assessments and assist navigation of resources.
  • Maintain knowledge of services and resources to access medical care and linkage to care.
  • Provide Naloxone training and distribution, Syringe Exchange Program, and related materials.
  • Maintain client files and complete required data entry.
  • Complete required training and stay updated on HIV/HCV/STI prevention trends.
  • Assist with program data collection, planning and reporting, and other administrative tasks.
  • Collaborate with the prevention team to maintain an inviting program environment.

Job description

JOB RESPONSIBILITIES:
  1. PREVENTION PROGRAM ACTIVITIES
    1. Coordinate and conduct outreach activities aligned with HIV Prevention and Harm Reduction best practices and community needs.
    2. In collaboration with the Advancement team and others, ensure the promotion and regular updates of Ryan White and HIV related information, current upcoming events, and necessary updates are posted online and/or promoted.
    3. Identify and conduct testing for individuals at high risk for HIV infection, increase education and awareness about prevention services and facilitate access to Counseling, Testing and Referrals (CTR).
    4. Provide CTR services for all clients during office visits or during outreach activities. Outreach activities may take place during evenings or weekends.
    5. Conduct Social Determinants of Health (SDOH) assessments at the prevention center or during outreach activities and assist navigation of resources within Sixteenth Street and other community agencies. Navigation services may include the provision of education, translation/interpretation, and other support based on client needs to facilitate access to services.
    6. Maintain a strong understanding of services and resources available to access medical care, treatment and other services including linkage to care, case management, and other resources in-house or at other agencies.
    7. Provide service to any individual visiting the Prevention Center, including Naloxone training and distribution, Syringe Exchange Program, safe use kits, and hygiene kits, among other services and materials available.
    8. Maintain files for each client participating in prevention interventions complying with State protocols and complete required data entry in the appropriate platforms.
    9. Complete all required training, and stay up to date on trends regarding HIV, HCV and STIs prevention, and other trends as identified by program management.
    10. Assist with program data collection, planning and reporting and other administrative tasks as determined appropriate by the Director of Community Health.
    11. Work closely with other members of the prevention team to successfully achieve the goals of the department while maintaining the organization of the Prevention Center to offer an inviting environment to our clients, this may include keeping supply inventories, procuring the cleanliness of office and work areas, and contributing with ideas for improvement of customer service experience.
  2. COMMUNITY ENGAGEMENT
    1. In collaboration with prevention program staff and other SSCHC staff, as needed:
      1. Connect with community partners, businesses, other stakeholders, and residents to develop partnerships and identify/create opportunities for collaborations.
      2. Maintain consistent familiarity with community resources and engage in meetings to better understand options to facilitate access for our clients.
      3. Monitor outcomes of referrals and assist in evaluating patterns of barriers that clients may face when attempting to access resources and work toward resolution as appropriate.
      4. Facilitate and/or participate in community events and meetings.
QUALIFICATIONS:
  1. Bachelor's or Associates degree in a health or social service-related field, with previous experience working with diverse populations and community based interventions preferred. Consideration will be made for a suitable combination of previous academic, personal and professional experience.
  2. Excellent oral and written communication skills. Bilingual (Spanish/English) fluency strongly preferred.
  3. Excellent organizational skills and ability to work independently and in a group setting.
