Job summary

Liberty Resources in Oswego, NY is seeking a full-time Prevention Caseworker to provide case management and counseling to identified youth and families under the Prevention Services program.

Under supervision, you will advocate, refer, coordinate services, and deliver group, individual and family counseling, including crisis intervention.

Qualifications

  • Bachelor’s degree and two years’ experience in school- or home-based settings providing services to individuals or groups.
  • Experience working with families and their needs.
  • Must have valid New York State driver’s license and access to reliable transportation.

Position Summary

Under the direct supervision of the Prevention Supervisor, the Caseworker is primarily responsible for providing case management and counseling services toidentifiedyouth and families.Responsibilities include advocacy, referral and linkage, group, individual and familycounselingand crisis intervention.

Essential Job Functions
  • Utilizingthe task centered case management model, implements and coordinates services to children and theirfamily'shaving troublein the home and/or in school, aimed at preventing placement of children outside the home and/or at improving academic performance.
  • Provides intervention and support to families in crisis intended to keep the family unit intact.
  • Completes all required documentation, in timelinesin accordance withprogram standards.
Duties and Responsibilities
  • Assesses children and families for risk, develops and implements service plans to address risk factors, drafts petitions and testifies in FamilyCourtas necessary.Provides Foster Care Services andlocatesplacements when needed.
  • Advocates on behalf of children and their families to ensure adequacy of services (i.e., educational, legal and community) by providing linkage and coordination with thecountyproviders and agencies.
  • Processes and reviews intake of all referrals to program.
  • Provides services in a manner which is culturally competent. Empowers clients by using clients’cultural strengths and informal support networks in service delivery.
  • Maintains effective professional relationships with other service providers and community organizations in the interest ofclients’and program goals.

  • Identifiesandreportson Untoward Incidents asrequired.
  • Carries out Agency policies and procedures.
  • Completes all necessary written reports, statistics, casefilesand summariesin a timely mannerandin accordance withprogram and agency standards.
  • Attends andparticipatesin individual supervisory conferences, staff meetings, trainingsessionsand professional development opportunities as required.Maintains required program certifications.
  • Provides door-to-door transportation as required.Ensuressafetyand welfare of passengers during loading,unloadingand transit.Make use of personalvehiclefor program transportation as required.
  • Handles emergency situations which may require flexibility of time and area of assignment.
  • Performs other duties,responsibilitiesand related tasks as assigned.
Qualifications

Bachelor’sand two (2)years’ experienceworking in a school- and/or home-based setting providing services to individuals and/or groups.Experience working with families and their needs.Must have valid New York State driver’s license and access to reliable transportation.

Required Skills and Abilities

Strong oral,writtenand public speaking communication skills.Must alsopossessstrong problem solving, decision making and conflict resolution skills andexhibita high levelofself-confidenceand responsibility.Must be able to work with complex systems (ex. Probation, Family Court, Foster Care) and have knowledge and/or experience with both the child welfare and juvenile justice systems.Ability to work independentlyand with flexibility indays and hours of work.Experience doing home-based work in a rural setting preferred.Must have access to reliable transportationand havea valid driver’s license.

Salary: $23.08 Per Hour
