Job summary

GBMC HealthCare seeks an LPN to provide direct patient care under the supervision of a registered nurse. The role requires effective communication with patients and families, and accurate documentation in the medical record.

You will collaborate with interdisciplinary team and participate in care planning and education. Responsibilities include data gathering, administering medications, preparing patients for tests, and reporting condition changes to the RN.

Qualifications

  • Current licensure as an LPN by the Maryland State Board of Examiners of Nurses.
  • Experience in acute care setting preferred.
  • Good communication with patients and families and ability to document accurately.

Responsibilities

  • Assist in gathering data to assess physical, psychological, social, spiritual needs of patients.
  • Collaborate with the RN and team to plan for patient needs on admission.
  • Provide direct patient care under supervision and document outcomes in medical records.
  • Administer medications and prepare patients for tests and procedures.
  • Report changes to the RN and participate in care plan updates.

Under direct supervision of the registered nurse provides direct patient care to assigned patients.EducationKnowledge of nursing theory and practice acquired through an accredited school of nursing.ExperienceOne-year acute care experience preferredSkills· Interpersonal skills, communication (verbal and written) are required in order to instruct and comfort patients and their families and maintain effective contacts with a variety of hospital personnel and maintain appropriate/adequate documentation.· Ability to perform and plan professional nursing care resolving complex patient care problems and dealing with patients and their families often during stressful situations.Licensures, Certifications· Current registration with the Maryland State Board of Examiners of Nurses as a Licensed Practical Nurse.· Healthcare Provider (CPR Certification) – not required for Hospice employee.Physical Requirements· Ability to stand and walk almost constantly (up to 90% of work time) and occasionally (up to 50% of work time) lifting and positioning patients.Working Conditions· Normal patient care environment with exposure to excessive noise, dust, and temperature.· Possibly exposed to adverse working conditions due to performance of patient care activities and exposure to communicable diseases.Principal Duties and Responsibilities:Assists in gathering data to assess the physical, psychological, social, spiritual needs of patients.Collaborates with the registered nurse and interdisciplinary/team members in planning for the needs of patients upon admission.Reviews the care plans and provides input into updating and revising.Supports in direct patient care to assigned patients utilizing clinical nursing skills and judgement.Provides education and reinforcement of education for patients and family, and documents outcome in the medical record.Initiates appropriate emergency measures when patient's condition dictates need.Prepares patients for tests and procedures.Administers and documents medications according to policy and procedure.Can appropriately delegate care to non-licensed personnel and report to the Registered nurse.Report to the Registered nurse with a thorough concise report on patient condition changes or evaluation of interventions.Perform other duties as assignedAll roles must demonstrate GBMC Values:RespectI will treat everyone with courtesy.
