Job summary

Concord Hospital Health System seeks an experienced Practice Manager for our Primary Care Practice in Franklin, NH, a Rural Health Clinic delivering primary care to rural communities.

Under supervision from the Director, the manager oversees daily operations, finances, clinical outcomes, program development and patient care, acting as the primary resource for staff and collaborating with hospital departments to sustain quality programs and services.

Qualifications

  • Associates degree with 5+ years of related experience or Bachelor's with 3+ years.
  • 2 years progressive supervisory experience.
  • Ability to lead a diverse, multi-age team.

  • Manages customer relations and customer service initiatives.
  • Oversees departmental core processes and supports organizational change.
  • Ensures compliance with regulatory requirements and professional standards.

  • Tracks meaningful use of metrics and provider dashboards.
  • Recruits, retains, and develops staff; manages performance.
  • Works independently and on cross-functional projects.

Job description

Seeking experienced Practice Manager for our Primary Care Practice located in Franklin, this is a Rural Health Clinic providing Primary Care Services in rural communities.

Summary

Under the general supervision of the Director, the manager is responsible for the daily management of the practice. This includes, but is not limited to operational, financial, clinical, performance improvement, program development, and customer relations and patient care outcomes. The manager is the primary resource person for staff. Responsibilities also include collaboration with other departments of the Hospital to assure development, promotion and maintenance of quality programs and services.

Education

Associates degree and minimum of 5 years of related experience or; Bachelors degree and minimum of 3 years of related experience.

Certification, Registration & Licensure

Meets Licensure/Certification requirements per department and/or professional standards.

Experience

Refer to Education 2 years progressive supervisory experience. Demonstrated ability to effectively lead a team as well as be a team member. Must possess knowledge specific for diverse age and multicultural population.

Responsibilities
  • Establishes positive working relationships with providers and acts as a conduit for departmental and organizational communication among staff and others.
  • Responsible for customer relations and customer service initiatives.
  • Manages departmental core processes, participates and supports departmental and organizational change.
  • Ensures compliance with State, Local, Federal regulatory requirements. Meets all departmental, professional and technical requirements.
  • Participates in establishing and monitoring annual department budget, accountable for development of action plans related to financial performance.
  • Analyzes financial performance on a daily, weekly, and monthly basis. Meets with department leadership to determine opportunities for improvement and associated action plans.
  • Understands key performance indicators and ensures targeted benchmarks are achieved in relation to Meaningful Use, Provider Dashboards, Revenue Cycle Metrics, etc.
  • Recruits, retains and develops staff and manages performance.
  • Takes responsibility for individual performance goals.
  • Works independently and within a team on special, nonrecurring and ongoing projects. Coordinates multiple aspects of projects, events, and other complex activities.
