Inova Health System in Falls Church, VA is seeking an experienced Nurse Practitioner or Physician Assistant for an outpatient post lung transplant role. The APP will work Monday–Friday with call, under physician supervision, and provide specialty care in an integrated team setting.

The role supports evidence-based practice, participates in quality initiatives, and contributes to cost-conscious patient management.

Qualifications

  • Licensed Physician Assistant or Nurse Practitioner with current board certification.
  • Eligible to practice in Virginia with an approved scope of practice.
  • Strong written and verbal communication; team collaboration.

Responsibilities

  • Practices in accordance with safe and effective standards of care and ensures compliance with professional and ethical standards of practice
  • Communicates with physicians, peers, nursing staff and other healthcare providers
  • Develops differential diagnosis and care plans based on data collection
  • Interprets laboratory, imaging and other data and reviews plan with patient and family
  • Evaluates response to care and health status, and promotes health optimization
  • Assumes clinical management for the specialty patient population with physicians
  • Maintains accurate history, physicals, progress notes and discharge summaries
  • Serves as a role model and preceptor for new APPs and students
  • Supports quality improvement initiatives and cost-conscious care
  • Contributes to standards development and participates in clinical research

Nurse Practitioner/Physician Assistant(APP) | Outpatient Post Lung Transplant (APP) | Inova Center for Personalized Health outpatient office

The Inova Outpatient Post Lung Transplant Team is looking for a Nurse Practitioner/Physician Assistant to join their team.

Outpatient APP role: Monday – Friday WITH call - (no weekends or holidays)

The Specialty Advanced Practice Provider exercises independent judgement in the assessment, diagnosis and initiation of medical processes and procedures, in accordance with the licensure regulations of the Commonwealth of Virginia, and as delineated in the written Scope of Practice and Protocol. Demonstrates respect for and an understanding of other clinical disciplines while using an integrated team-based approach to achieve desired patient outcomes. Utilizes evidenced based standards, guidelines and protocols for care delivery. Participates in quality and best practice initiatives by utilizing standards and guidelines for cost effective care delivery to continuously improve patient care and enhance outcomes. Performs high-quality clinical practices and effective patient management under physician supervision by following state regulations, including primary and secondary specialty care in clinical practice settings.
Job Responsibilities

  • Practices in accordance with safe and effective standards of care and ensures compliance with professional and ethical standards of practice
  • Communicates appropriately with physicians, peers, nursing staff and other healthcare providers
  • Develops differential diagnosis and plans of care based on data collection and revises based on physiologic and psychosocial findings
  • Interprets and synthesizes laboratory, imaging and other pertinent data and reviews patient plan of care with interdisciplinary team and with the patient and family
  • Evaluates and documents patients' response to care, and evaluates impact of illness on patients' health status, functional status, nutritional status, sleep and rest patterns, family and social relationships, and establishes a health promotion strategy
  • Assumes clinical management for the specialty patient population in collaboration with physicians in practice
  • Provides accurate record keeping as evidenced by complete and pertinent history and physicals, pre-operative and progress notes, discharge summaries and data information sheets
  • Serves as a role model and preceptor for new Advanced Practice Providers and students
  • Participates in and supports the departmental and system-wide quality performance improvement initiatives
  • Supports departmental cost savings strategies and provides care in an efficient and cost conscious manner
  • Assists in the development and revision of standards for patient populations and integrates evidence-based research findings into clinical practice
  • Participates in clinical research and data collection and assists in projects and planning for continual development of respective specialty
  • Provides clinical and professional educational programs to improve staff knowledge
  • Proactively assists peers with work tasks
  • May perform additional duties as assigned

Minimum Qualifications:

  • Certification- Basic Life Support upon start; Advanced Cardiovascular Life Support (for Critical Care) upon start
  • Licensure- Licensed Physician Assistant or Nurse Practitioner; If Nurse Practitioner: RN Compact License(depending on State of primary residence) AND a license (or eligibility for) as a nurse practitioner or physician assistant in the Commonwealth of Virginia AND a "Practice Agreement" signed by sponsoring/ supervising physician(s), AND a written "Scope of Practice/Protocol" signed by the supervising physician, AND "Drug Enforcement Administration License" (if prescribing controlled substances)
  • Board Certification -Physician Assistant or Acute Care Nurse Practitioner. Maintains necessary CME requirements.
  • Education- Bachelor's in Nursing and Master's in Advanced Clinical Practice or graduate of a post masters certificate program for nurse practitioners, OR a certificate from an education program that is accredited by the Council of Nurse Anesthesia Educational Programs/Schools, American College of Nurse Midwives, American Association of Colleges of Nursing of the National League for Nursing. For Physician Assistants, a graduate of an accredited Physician Assistant program and certification by National Commission on Certification of Physician Assistants

Preferred Qualifications: prior APP transplant experience preferred, any organ.
