Job summary

TeamHealth seeks an Advanced Practice Clinician in Manchester, CT, to join our post-acute care team. This full-time position provides rounding in skilled nursing facilities with strong support, autonomy, and earnings potential.

Eligible for onboarding and mentorship as part of our clinical practice. You will deliver high-quality care to diverse patients, collaborate with a seasoned team, and contribute to meaningful patient relationships and family education.

Qualifications

NP or PA license (CT) and DEA registration.
Post-acute, acute, ED, or clinic experience preferred; new grads welcome to apply.
Strong clinical foundation and knowledge of healthcare regulatory standards.
Excellent communication and interpersonal skills.
Collaborative, team-oriented approach with a positive outlook.

Responsibilities

Perform comprehensive patient assessments, including medical histories and physical exams.
Develop, implement, and adjust individualized care plans based on patient needs.
Monitor progress and coordinate care to optimize outcomes.
Collaborate closely with physicians, nursing staff, and other healthcare professionals in a team approach.
Educate patients and families on health management and preventive strategies.
Maintain accurate, timely medical records in compliance with regulations.
