Job summary

Altea Healthcare in Lonaconing, MD, seeks a Nurse Practitioner to join our post-acute care team as a full-time provider. You will diagnose, treat, and coordinate care for patients in a skilled nursing facility, delivering high-quality, patient-centered care with a focus on geriatrics and chronic disease management.

Qualifications

• NP License (or eligibility to obtain).
• All experience levels welcome; training available.
• Geriatrics, internal medicine, or primary care passion.
• Self-motivated with patient-first approach.
• Strong communication and teamwork.

Responsibilities

• Performing physical exams and reviewing medical histories.
• Ordering and interpreting diagnostic tests (labs, imaging, etc.).
• Diagnosing and managing acute and chronic conditions.
• Prescribing medications and creating treatment plans.
• Collaborating with physicians, nurses, and facility staff.
• Educating patients and families on health conditions and preventive care.
• Documenting patient care accurately and efficiently.
