Job summary

You will collaborate with nursing, therapy, physicians, social services, and external providers, document activities in the EMR, and ensure timely transfers and coverage for post-acute services. This is a per diem, on-site role in a hospital setting in California.

Qualifications

  • Bachelor's degree in healthcare, social services, human services, or related field preferred.
  • Two years of experience in healthcare or case management support preferred.
  • American Heart Association BLS certification required or obtained within the timeframe established by department policy.

Responsibilities

  • Coordinate discharge planning and transitions for IRU and Westland House patients.
  • Gather information on living situation, barriers, and transportation for discharge.
  • Coordinate arrangements for home health, durable medical equipment, and community services.
  • Track discharge dates, referrals, and authorizations; escalate delays.
  • Participate in IRU and Westland House care conferences and maintain EMR documentation.
  • Serve as a consistent contact for routine administrative coordination during discharge and transitions.
