Puzzle Healthcare in Crown Point, IN is seeking a Nurse Practitioner or Physician Assistant for an onsite post-acute care role serving SNFs. You’ll round on weekdays, conduct focused micro visits, and work with physicians, therapists, and staff to support recovery.

You’ll document in Puzzle’s EHR (often remotely from home), monitor readmission risk, and ensure compliance with post-acute and PM&R regulations.

Responsibilities

• Round in assigned facilities on a consistent weekday daytime schedule
• Conduct focused micro visits to assess recovery, pain, wounds, and function
• Collaborate with physicians, therapists, and staff on recovery plans
• Document in Puzzle’s EHR, often remotely from home
• Monitor readmission risk and close gaps in post-acute care
• Maintain compliance with post-acute and PM&R regulations
