Job summary

Qualifications

• Graduate of an accredited physician assistant or nurse practitioner program.
• Current national certification as physician assistant or nurse practitioner.
• Must have prior experience working in post-acute.
• Eligible for privileges to post-acute facilities.

Responsibilities

• Evaluates patients by history and physicals; interprets labs and tests for acute and chronic conditions.
• Collaborates with nurse practitioners or supervises physicians in developing treatment plans.
• Orders and interprets diagnostic tests; prescribes medications as allowed.
• Facilitates transitions between care settings and coordinates referrals and admissions.
