Responsibilities

  • Obtain vital signs and support screening procedures (e.g., IRIS, PVD, A1C, urine microalbumin).
  • Coordinate preventive screenings and connect patients with internal care management and support services.
  • Assist with operational workflows, including preparing provider materials, running reports, maintaining records, and updating system data.
  • Support Population Health initiatives, health fairs, and program activities to drive quality outcomes and patient compliance.

Position Purpose

The Population Health Technician (Medical Assistant) supports Population Health initiatives through direct patient interaction, clinical support, and coordination of care activities.

This role partners with Nurse Practitioners and Physician Assistants to facilitate in-home, clinic, and telehealth visits, while helping drive patient engagement, preventive care, and improved health outcomes.

How You Contribute to the Company’s Mission
  • Support Population Health visits by gathering patient information, completing screenings, documenting in the EMR, and assisting providers in home, clinic, and telehealth settings.
  • Conduct patient outreach, including scheduling visits and follow-up calls to support patient engagement and improve health outcomes.
  • Perform clinical tasks such as obtaining vital signs and supporting screenings (e.g., IRIS, PVD, A1C, urine microalbumin) within scope of practice.

Required Qualifications

Preferred Qualifications
  • Certified or Registered Medical Assistant (CMA/RMA) credential.
  • Experience in a clinical, Population Health, or in-home care setting.
  • Experience performing patient triage and basic clinical procedures.
  • Familiarity with telehealth platforms and hybrid care delivery models.
  • Experience working with multiple data systems and healthcare workflows.
  • Demonstrated ability to multitask and prioritize in a dynamic environment.

Perform other job-related duties as assigned, within your scope of responsibilities. Job duties are performed in a professional office, clinic, or patient home environment. Frequent travel within the assigned region is required. Work includes standing, walking, and extended periods of sitting, with regular use of standard medical and office equipment. The employee must be able to lift and/or move up to 30 pounds and safely navigate various environments, including patient homes. Effective communication, documentation, and the ability to adapt to varying care settings are required.
