Blue Cross and Blue Shield of Louisiana Inc. seeks a Population Health Technician (Medical Assistant) to support in-home, clinic, and telehealth visits.
The role collaborates with Nurse Practitioners and Physician Assistants to engage patients in preventive care, document in EMR, and coordinate care activities across settings. The position requires a high school diploma, a Louisiana driver’s license, and experience in clinical or patient-facing environments.

The Population Health Technician (Medical Assistant) supports Population Health initiatives through direct patient interaction, clinical support, and coordination of care activities.
This role partners with Nurse Practitioners and Physician Assistants to facilitate in-home, clinic, and telehealth visits, while helping drive patient engagement, preventive care, and improved health outcomes.
Perform other job-related duties as assigned, within your scope of responsibilities. Job duties are performed in a professional office, clinic, or patient home environment. Frequent travel within the assigned region is required. Work includes standing, walking, and extended periods of sitting, with regular use of standard medical and office equipment. The employee must be able to lift and/or move up to 30 pounds and safely navigate various environments, including patient homes. Effective communication, documentation, and the ability to adapt to varying care settings are required.
