Job summary

Peninsula Behavioral Health in Port Angeles, WA seeks a Population Health Registered Nurse to join our interdisciplinary team. You will help individuals with chronic medical and behavioral health conditions improve health, manage symptoms, and gain greater independence in the community.

Qualifications

• Current Washington State RN or LPN license.
• Graduate of an accredited nursing program.
• Minimum one year of clinical nursing experience.
• Valid Washington driver's license, reliable transportation, and proof of insurance.

• Conduct outreach and engage clients in healthcare services.

• Support self-management goals using motivational interviewing techniques.
• Collaborate with providers, health plans, community partners, and care teams.
• Assist with transitions of care and population health initiatives.
• Participate in crisis intervention, outreach services, and occasional home visits as needed.
• Maintain accurate documentation in the electronic medical record (EMR).

Peninsula Behavioral Health is seeking a compassionate and motivated Population Health Registered Nurse to join our interdisciplinary team. In this role, you'll help individuals with chronic medical and behavioral health conditions improve their overall health, manage symptoms, and achieve greater independence in the community.

What You'll Do

Preferred:

• Experience working with individuals experiencing chronic mental illness and complex medical needs.
