Rochester Regional Health is seeking a Population Health Coordinator to join the Population Health department at the Riedman Campus in Rochester, NY. The role supports care coordination, patient outreach, and scheduling to improve population health outcomes.
It requires strong communication, basic computer skills, and familiarity with medical terminology. The position is full-time, on-site, with a 40-hour work week and standard weekday hours.
Key member of the practice team who has an understanding of primary care practices and assists in the coordination of care required for optimal coordinated population health.
