Job summary

Ideal candidates hold a Bachelor’s in Nursing and RN licensure in NJ/PA, with 5+ years in acute care/managed care settings. Travel to practices is required.

Qualifications

• Bachelor’s Degree in Nursing required; RN license in NJ and PA.
• Five years clinical experience in acute care, rehabilitation, sub acute, home care, managed health plan, or outpatient setting preferred.
• AHA BLS - Healthcare Provider and RN licensure required.

Responsibilities

• Coordinate identified patient care with primary and specialty care and external agencies.
• Monitor transitions of care after hospital admissions and ER visits.
• Perform follow-ups to assess medical, psychosocial, and functional status of high-risk patients.
• Utilize EMR/registries to prioritize follow-ups and track care metrics.
• Coach patients to ensure access to medications and services per care plans.
• Identify high-risk patients and implement best-practice processes for preventive and chronic care.
