Job summary
Upperline Health in Carmel, IN seeks a Nurse Practitioner to deliver clinic-based care in collaboration with the physician team. APPs develop complex care plans, provide diagnostics and treatments, and educate patients across clinic, home, or telehealth settings.

Qualifications
- Master’s degree in nursing and NP board certification required.
- Current RN license and BLS certification required.
- Experience in podiatry, wound care, or home-based care preferred.
- DEA registration or eligibility desirable.
- Experience with AthenaHealth EMR and Microsoft Office.
- Strong communication and interdisciplinary collaboration skills.
Responsibilities
- Provide clinic-based NP care collaborating with physician team.
- Perform podiatry-related foot care and wound care within scope.
- Develop and implement plans of care for adults with chronic conditions.
- Document in electronic medical records and coordinate with care teams.
- Educate patients and families on med usage, diet, and prevention.
- Participate in weekly interdisciplinary meetings and continuing education.
