- Experience working in nursing homes is preferred.
Responsibilities
- Work in partnership with staff and facility leadership.
- Provide compassionate care for patients in our facilities.
- Collaborate care with the patient’s healthcare team.
- Attend facility meetings as necessary.
- Complete all documentation in EHR.
