Job summary

Sweetwater Care in California is seeking an LVN to provide nursing care within scope of practice, following physician orders, center policies, and state guidelines. The role involves medication administration, treatments, patient and family education, and collaboration with physicians during rounds.

The ideal candidate is licensed in California, has long-term care experience, and familiarity with PointClick Care charting. Prior nursing management experience is preferred.

Qualifications

Responsibilities

• Utilizes Infection Control Manual to guide practice.
• Participates in and supports Quality Improvement programs.
• Identifies and monitors patients in appropriate programs per Quality Life Manual.
• Notifies departments of patient changes (room changes, LOA, admissions, discharges).
• Conducts family and patient education.
• Administers medications timely and per facility policy.
• Administers treatments timely and per facility policy.
• Receives, transcribes and carries out physician orders when allowed by Nurse Practice Act.
• Might participate in rounds with physicians.

Job description

Objective

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.

Principal Responsibilities

• Demonstrates knowledge and appropriate use of the Company Infection Control Manual.
• Understands, participates and assists in implementing the Quality Improvement Program.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Notifies other Departments of patient changes, i.e., room changes, LOA, new admits, discharge, etc.
• Conducts family and patient education.
• Demonstrates the ability to administer medications timely and according to facility policy.
• Demonstrates the ability to administer treatments timely and according to facility policy.
• Demonstrates ability to receive, transcribe, and carry out physician orders, if allowed by Nurse Practice Act
• Makes rounds with physicians when necessary.

Qualifications

• Currently licensed as LVN in state of California
• One-year prior nursing management experience preferred, not required.
