CommuniCare Family of Companies is seeking a dedicated Nurse Practitioner to join our team on a full-time basis at our Blue Ash, Ohio facility. The role focuses on physiatry within inpatient rehabilitation and long-term care settings, with PM&R exposure provided if needed.

You will assess, diagnose, and treat patients under physician supervision while collaborating with an interdisciplinary team to deliver high-quality, person-centered care.

Formación

• Master’s degree from an accredited NP program.
• Current NP license in the state of West Virginia.
• Minimum of two years of clinical experience preferred.
• Strong clinical assessment and diagnostic skills.
• Excellent communication, collaboration, and interpersonal abilities.
• Commitment to providing compassionate, patient-centered care.

Responsabilidades

• Provide direct patient care, including assessment, diagnosis, and treatment, under the supervision of the attending physician.
• Collaborate with the interdisciplinary team to develop and implement care plans for inpatient rehabilitation and long-term care patients.
• Conduct regular rounds and follow-up visits to monitor progress and adjust treatment plans.
• Educate patients and families about their medical conditions, treatment options, and self-care techniques.
• Participate in clinical meetings, case conferences, and quality improvement initiatives.
