Job summary

Iconic Care seeks a passionate physiatrist to provide autonomous care at contracted skilled nursing facilities in California. You will round at two facilities, focusing on enabling therapy participation and functional recovery for 25+ patients daily.

You will perform injections, manage medications, and collaborate with facility primary care teams. This role offers a high degree of autonomy, ongoing professional development, and a commitment to improving rehabilitation outcomes.

Qualifications

• Current, unrestricted PA or NP license.
• DEA license or willing to obtain DEA licensure.
• Nursing home and/or physiatry experience is a plus, but not required.
• Comfort with peripheral joint injections (knees, shoulders, hips).
• Autonomous environment with professional, personalized care.
• Strong communication with multidisciplinary teams and EHR charting.

Responsibilities

• Round at two SNFs or similar rehab centers, 2–3 days per location weekly.
• Develop and execute individualized treatment plans with facility teams.
• Perform joint injections and related procedures as needed.
• Be the sole physiatrist on-site with a collaborative clinical team for consultation.
• Coordinate care and referrals to other specialties as needed.
