Shadeland Avenue Health Center in Indianapolis, IN, is seeking a Med-Peds physician to provide comprehensive outpatient primary care to patients of all ages within a mission-driven Federally Qualified Health Center.

The physician will care for pediatric, adolescent, adult, and geriatric patients in a collaborative, integrated care model with strong support for behavioral health and care management, focused on improving access to care for underserved communities.

Qualifications

• MD or DO is required.
• Board Certified in Pediatrics and Internal Medicine.
• Eligible for or currently holding an Indiana medical license.
• DEA registration (or ability to obtain).
• Commitment to patient-centered, community-focused care.

Responsabilités

• Provide outpatient primary care across the lifespan.
• Diagnose and manage acute and chronic conditions.
• Deliver preventive care, wellness visits, and immunizations.
• Collaborate with specialists, behavioral health providers, and care teams.
• Support quality improvement and population health initiatives.
