UPMC Central PA is seeking full-time Adult and Senior Care Physicians to join our Post-Acute Care team in the Hanover region of South Central Pennsylvania. You will care for geriatric patients across skilled nursing facilities, rehab, and clinic settings, collaborating with facility leadership to ensure high-quality care.

Qualifications

• Board-certified/board-eligible in Primary Care, Internal Medicine, Family Medicine or Geriatrics.
• Licensed to practice medicine in Pennsylvania.
• Experience with geriatric populations and post-acute care settings.

Responsibilities

• Provide direct patient care across skilled nursing, rehab, and clinics.
• Collaborate with discharge teams and care management to drive quality outcomes.
• Supervise Advanced Practice Providers (APPs).
• Use EPIC EMR; access PACS imaging and labs.
• Clinical rounds M-F with flexible scheduling; equitable call coverage.

About the Position:

• Provides direct patient care in multiple facilities across the aging continuum to include skilled nursing, rehab, personal care/assisted living, independent living and clinic.


• Interacts with facility leadership, hospital discharge team, care management and other specialties to drive optimal quality care with aligned goals.


• Post-Acute care offers a wide variety of healthcare fields, from general primary care and internal medicine to geriatrics and acute rehab services. Our clinician's crossover and work with several specialists, increasing their knowledge base and skillsets.


• Collaborates with and supervises Advanced Practice Providers.


• EPIC EMR, online diagnostics including PACS imaging and laboratory results.


• Clinical rounding M-F, attractive daily schedules with flexible models. Overnight and weekend call is all phone coverage, shared equally with partners and APPs, ratios very favorable.
