Job description

As a key member of the interdisciplinary care team, the physician in this role provides medical care for geriatric and other adult patients in our Skilled Nursing Facility (SNF) and post-acute rehabilitation units. This position supports a collaborative, team-based model of care and includes teaching responsibilities with medical students, residents, and fellows affiliated with Harvard Medical School.

Core Competencies

The ideal candidate will:

  • Deliver comprehensive and goal-concordant care to post-acute and long-term patients. Demonstrate proficiency in inpatient medical care and familiarity with SNF regulatory standards. Collaborate effectively with interdisciplinary teams including nursing, therapy, pharmacy, social work, and case management. Mentor and supervise learners in the care of older adults. Contribute to quality and safety initiatives across the care continuum. Exhibit strong communication, organizational, and leadership skills.
Key Responsibilities
  • Perform medical evaluations, assessments, diagnoses, and treatment for patients in SNF and post-acute care. Prescribe medications and develop individualized care plans aligned with patient goals. Refer patients to specialists and coordinate care as appropriate. Document patient encounters in a timely and accurate manner using the EMR system. Participate in overnight, weekend, and holiday call as assigned. Lead or participate in interdisciplinary team meetings and care planning. Collaborate in quality improvement and infection control initiatives. Support education programs through teaching and supervision of medical trainees. Attend regular medical staff meetings and contribute to departmental initiatives. Fulfill additional duties aligned with the mission and needs of Hebrew SeniorLife.
Qualifications
Education:
  • MD or DO degree. Completion of an internal medicine or family practice residency (minimum three years).
Licensure and Certification:
  • Valid and unrestricted Massachusetts medical license. Board certified or board eligible in Internal Medicine or Family Medicine. Current Federal DEA registration. CPR/BLS certification required.
Experience:
  • Clinical experience providing care for seniors in skilled nursing or post-acute care settings. Proficiency in managing complex chronic conditions and post-hospitalization transitions. Strong understanding of regulatory, safety, and quality measures in SNFs. Experience working in and/or leading interdisciplinary clinical teams. Commitment to continuous learning, patient-centered care, and educational mentorship.
