Job summary

Twin Cities Physicians seeks a Mobile Primary Care Physician to provide on-site geriatric rounding and primary care across assisted living, memory care, and skilled nursing facilities in Minnesota. You’ll manage complex patients, coordinate with a diverse healthcare team, and aim to prevent hospitalizations while enhancing quality of life.

The role emphasizes proactive care delivery, early intervention, and collaboration with NPs, PAs, nurses, and therapists in a senior living community setting.

Qualifications

• Active Minnesota Physician license in good standing.
• Experience in geriatrics, internal medicine, family medicine, hospital medicine, or post-acute care.
• Strong clinical judgment and ability to manage medically complex patients.

Responsibilities

• Provide on-site primary care and geriatric rounding across care settings.
• Manage chronic conditions, transitional care needs, and preventive care.
• Identify concerns early and implement interventions to improve outcomes.
• Reduce hospitalizations and ER utilization through proactive care.
• Perform diagnostics, treatment planning, and medication management.
• Collaborate with nurses, PAs, and inter-disciplinary teams.
• Maintain EMR documentation and care coordination.

As a Mobile Primary Care Physician with Twin Cities Physicians, you'll focus on identifying risks early, managing medically complex patients, and helping seniors remain healthier within the communities they call home.

You’ll work across assisted living, memory care, skilled nursing, and long-term care environments, helping prevent avoidable hospitalizations while improving quality of life for a highly vulnerable patient population.

Responsibilities

• Provide on-site primary care and geriatric rounding throughout assisted living, memory care, skilled nursing, and long-term care communities
• Manage chronic conditions, transitional care needs, preventative care initiatives, and medically complex patient populations
• Identify clinical concerns early and implement interventions that improve outcomes
• Reduce hospitalizations and emergency room utilization through proactive care delivery
• Perform diagnostics, treatment planning, medication management, and follow-up care
• Collaborate with Nurse Practitioners, Physician Assistants, nursing staff, therapists, and case managers
• Maintain accurate EMR documentation, care coordination, and ongoing clinical oversight

Qualifications

• Active Minnesota Physician (MD or DO) license in good standing
• Experience in geriatrics, internal medicine, family medicine, hospital medicine, post-acute care, or related specialties preferred
• Familiarity with chronic disease management, transitional care, or value-based care models is beneficialStrong clinical judgment and confidence managing medically complex patient populations
• Valid driver's license and reliable transportation
