Job summary

FIND Healthcareers is seeking a licensed physician (MD/DO) to lead in-home and telemedicine care for patients in Arizona. This role involves supervising up to four PAs/NPs, signing charts, and delivering comprehensive primary care in home settings.

You will diagnose, treat, and develop care plans for patients across private homes and assisted living facilities, using evidence-based guidelines and clear patient education. Telemedicine is a key component of care delivery.

Essential Functions

• - Oversee up to four Physician Assistants or Nurse Practitioners, including signing off on patient charts.
• - Diagnose and treat patients in private homes or assisted living facilities, including House Call services for common chronic diseases via “in-person” House Call and/or telemedicine visits
• - Analyzing and interpreting patients' histories, symptoms, physical findings, or diagnostic information to develop appropriate diagnoses
• - Developing treatment plans based on standards of care and professional practice guidelines
• - Chronic Medication management and educating patients on test results and treatment plans
• - Ordering or performing diagnostic tests.

Requirements

• - Doctor of Medicine Degree (MD) or Doctor of Osteopathy Medicine Degree (DO)
• - Licensed as a physician in good standing in Arizona , additional states preferred
• - Board-certified in Family Medicine or Emergency Medicine
• - 2+ years of post-residency experience in primary care, family practice, urgent care or specialties
• - Outstanding clinical expertise, evidenced-based practice
• - Culturally responsive and competent, providing patient education at every visit
• - Strong verbal and written communication skills
• - Above average comfort with technology and assisting patients with troubleshooting technical difficulties that may occur in the home environment
