Job summary

Prisma Health in Sumter seeks an experienced physician to deliver direct primary medical care to program participants, including frail and elderly individuals, in the Tuomey area.

You will perform histories and physicals, manage episodic illnesses, coordinate referrals and hospital admissions, and lead discharge planning as part of a collaborative care team. This role supports nursing home care with regular visits and coordination with family conferences.

Qualifications

• MD or DO from an accredited medical school; substitutions not allowed.
• Minimum 2–3 years of experience, including at least 1 year with frail/elderly population.
• Current license to practice medicine in SC and BLS certification.

Responsibilities

• Perform comprehensive history and physicals on new referrals.
• Provide care for episodic illnesses and coordinate with specialists.
• Manage hospital admissions and discharge planning with the care team.
• Deliver regular nursing home visits and telephonic check-ins as required.
• Participate in family conferences and team meetings.

Job Summary

Provide direct primary medical care to all participants of the program.

Accountabilities

• Performs comprehensive history and physical on new referrals.

• Performs interval history and physicals on established participants completed quarterly or as indicated.

• Evaluates and treats participants with episodic illnesses

• Refers participants to medical specialist as indicated.

• Admits participants to the hospital: (a) Provides primary care (attending) responsibilities for management; (b) Provides updates to the team on a regular basis; (c) Judiciously utilizes specialty consultants; (d) Is responsible for coordinating discharge planning with the team.

• Manages all care of participants in the nursing home: (a) Provides regular visits; (b) Performs telephone contacts with nursing home staff as required; (c) Admits nursing home participants to hospital when necessary.

• Actively participates in family conferences.

• Participates in team meetings, committees and work groups as assigned.

Supervisory/Management Responsibility

This is a non-management job that will report to a supervisor, manager, director, or executive.

Minimum Requirements

• Doctor of Medicine (M.D.) or Doctor of Osteopathic Medicine (D.O.) from an accredited school required. Work experience may NOT substitute for education requirement

At least 2-3 years’ experience, minimum one year experience working with a frail/elderly population.

Required Certifications/Registrations/Licenses

Current license to practice medicine in SC; BLS Required.
