Banner Health in Queen Creek, AZ is seeking a Board-Certified/Board Eligible Family Medicine or Internal Medicine physician to join our expanding outpatient practice. You will work in a physician-led group with strong support, focusing on chronic disease management, Medicare wellness, and well-child care in a value-based setting.

• 36 hours of patient contact and 4 hours admin time per week.
• Support includes Medical Assistants, integrated specialty team, PharmD, onsite labs, and additional resources.

Our clinic is currently focused on primary care services with chronic disease management, Medicare wellness, physicals, well-child examinations, acute care visits and minor in-office procedures.

Position Highlights:

• Emphasize quality outcomes through collaborative care with our population health team

• Monday through Friday, with occasional Saturday

• Light call: phone call consult only

• 36 hours of patient contact; 4 hours admin time
