Job summary

Open Door Community Health Centers in Fortuna, CA seeks a Primary Care Physician to join our integrated, mission-driven team at Fortuna CHC. You will provide outpatient, patient-centered care across the lifespan in a rural FQHC, diagnosing and managing acute and chronic conditions while emphasizing preventive care and care coordination with an interdisciplinary team.

Qualifications

• Successful completion of accredited medical school (MD Degree Required).
• Completion of accredited Internal or Family Medicine Residency Program required.
• Current, unrestricted California physician and surgeon license.
• Current DEA registration.
• Current Board Certification in specialty of practice (or board eligible if a newer graduate).
• Current CPR/BLS certification.
• Valid California driver's license with an insurable driving record.

Responsibilities

• Provide comprehensive outpatient care for a patient panel across the lifespan in a rural FQHC.
• Diagnose, treat, and manage acute and chronic conditions; emphasize preventive care.
• Coordinate specialty services when needed; work with interdisciplinary care team.
