Job summary

HealthLinc in Indiana seeks a physician to coordinate proactive care for patients with chronic conditions and to identify wellness and preventive services for clinical patients. You will work closely with the clinical staff and report to the Site Medical Director, ensuring comprehensive care and adherence to PCMH standards.

You will develop and manage care plans, coordinate scheduling and follow-up, and provide leadership in supervising advanced practice nurses when assigned.

Qualifications

  • Graduation from an approved medical school.
  • Completed Residency Program.
  • Board certification in a medical specialty.
  • Indiana State Medical License.
  • DEA certification and CPR certification.
  • Ongoing continuing medical education as required.

Job description

As a Physician, you will be responsible for coordination of proactive care for patients with chronic conditions as well as identifying wellness and/or preventative service for clinical patients. This position will work closely with the clinical staff and will report to the Site Medical Director.JOB RESPONSIBILITIES:Utilizes appropriate and effective patient care plans which includes adequate diagnostic procedures, therapies, use of consultation and referrals, and outcomes.Manages patients to assure continuity of care which includes adequate scheduling and tracking of return visits, completion of treatment plans, and follow-up of chronic problems, abnormal tests, and no show appointments.Provides whole-person care which includes appropriatereferrals to the social worker, nutritionist, and mental health professionals.integration of biologic, social, psychological and environmental issues.Maintains adequate, concise, and legible documentation of patient care.Provides and documents appropriate periodic health examinations. (i.e. preschool vision exams, serum cholesterol, pap smears, etc.)Educates patients about their diagnosis, treatment, prognosis, medications, preventive care, etc.Utilizes appropriate infection control measures, including universal precautions.Supervises advance practice nurses in Clinic when assigned.Provides consultation and prescription signatures to advance practice nurses when needed.Oversees residents in the management of hospital patients.Writes daily progress notes in hospital charts.Takes phone calls about hospital patients.Participate in assigned Quality Assurance/Management in Daily Work activities.Regularly attends QA/TQM as required.Participates in the physician performance audit as assigned.Assists in the development and maintenance of HealthLinc practice guidelines (protocols) as assigned.Participates in Total Quality Management (TQM) activities.Attends Clinics Provider Group Meetings and hospital section meeting as scheduled.Attends required in-service meetings.Makes presentations in the community when requested.Teaches students and residents as assigned.All HealthLinc staff is committed to engage in quality improvement initiatives that align with and support Patient-Centered Medical Home (PCMH).Performs other duties as assigned.REQUIRED QUALIFICATIONS:Education/TrainingGraduation from an approved medical schoolCompleted Residency ProgramBoard certificationDEA certificationCPR certificationIndiana State Medical LicenseContinuing medical educationExperienceAt least 1-3 years of experience as a physician (not required but highly preferred)Skills/Job RequirementStrong customer service skillsStrong organizational and time management skillsProven ability to work well in a team environmentExcellent written and verbal communication skillsAbility to remain flexible and adaptableTransportation and valid drivers licenseTechnology SkillsOperate a multi-line phone system and other office equipment including printers, fax machines, etc.Basic software skills (Microsoft Office, EHR, online sources, etc.)DIRECT SUPERVISION:N/AREQUIRED TRAINING:All assigned Realis trainings
