Job summary

Form Health, a virtual obesity medicine clinic, seeks obesity medicine physicians to provide longitudinal telehealth care for patients with obesity. Work is remote with flexible schedules, and candidates must be licensed in MA, NY, or VA.

The role includes participation in a rotating after-hours schedule with leadership and protected time for administrative duties. This is a Full Time (W2, 40hrs/week) position based in a remote telemedicine setting, with a supportive care team and opportunities

Qualifications

• MD or DO degree with board certification (ABOM or Endocrinology).
• Licensed to practice in MA, NY or VA.
• Multi-state practice capability (LOQ/IMLC).
• Credentialed with insurance payers; multiple state licenses preferred.
• Experience conducting telemedicine visits and strong web-side manner.
• Bilingual in Spanish preferred.

Responsibilities

• Provide direct patient care in obesity medicine via telemedicine.
• Current role does not involve APP collaboration; future chart reviews for APP patients.
• Develop treatment plans and medical management for complex cases.
• Prescribe non-controlled obesity medications under direct care.
• Ensure documentation, protocols, and care pathways are followed within the team.
• Support professional development of APPs and supervise if needed.
• Participate in team-based care initiatives and quality improvement.
• Attend departmental meetings and contribute to program improvements.

About the Role:

We are seeking obesity medicine physicians to provide longitudinal telehealth care for patients with obesity, and to collaborate with advanced practice professionals and registered dietitians towards exceptional team-based care. Work is remote, and schedules can be flexible.

We are currently accepting applications for a FTE (40 hours per week) Obesity Medicine Physician or Endocrinologist opportunity, who are licensed in MASSACHUSETTS (MA), NEW YORK (NY) or VIRGINIA (VA).

This role will include participation in a rotating after hours call schedule with Form Health clinical leadership and will have a reduced clinical schedule the weeks of on-call coverage.

This is a Full Time (W2, 40hrs/week) position based in a remote, telemedicine clinical setting. The physician will work closely with a supportive care team and have protected time for administrative duties.

What you will do:

• Provide direct patient care in a clinical setting, focusing on evidence-based obesity treatment and chronic weight management. The physician will have their own panel and will collaborate with Registered Dietitians.
• At this time, this role does not collaborate with APPs. In the future, this physician role will be responsible for chart reviews and conduct return visits for APP patients every third visit to ensure quality and continuity of care.
• Provide treatment planning and medical management support for complex cases or escalated concerns.
• Prescribe non-controlled obesity medications for patients under direct care
• Ensure clinical documentation, protocols, and treatment pathways are followed consistently within the team.
• Assist in the professional development of APPs by fostering clinical growth, offering feedback, and supporting continuing education. This role could include APP collaboration and supervision responsibilities.
• Participate in team-based care initiatives and contribute to a culture of collaboration and high-quality care.
• Attend relevant departmental meetings and contribute to quality improvement efforts within the obesity medicine program.

About You:

• MD or DO degree with board certification; must be an American Board of Obesity Medicine (ABOM) diplomate or board certified in Endocrinology
• Are licensed to practice and prescribe legally in MASSACHUSETTS (MA), NEW YORK (NY) or VIRGINIA (VA).
• This position requires multi-state practice capability. Applicants must either already hold a valid Letter of Qualification (LOQ) through the Interstate Medical Licensure Compact (IMLC) or meet all prerequisites to rapidly secure one upon hire
• Are or have in the past credentialed with insurance payers (strong preference for multiple state licenses!)
• Have excellent interpersonal skills for patient and team interactions
• Are committed to the highest quality of medical care and an evidence-based approach to obesity management
• Have prior experience conducting telemedicine visits and who have a great “web-side” manner
• Have prior experience working collaboratively with advanced practice professionals and registered dietitians
• Bilingual candidates with medical fluency in Spanish are highly preferred

Base Annual Salary

$210,000—$230,000 USD
