You will provide wholistic care including prevention, chronic disease management and health education to patients, practicing autonomously with resources like UpToDate and RubiconMD.

The role emphasizes patient-first care and collaborative team learning, with a focus on high-quality outcomes and cost-effective care within a value-based model.

Qualifications

• Active license and current NCCPA board certification required.
• Independent practice provider preferred, where applicable per State regulation.
• Federal DEA number preferred; may be required for full prescribing ability.
• CPR/BLS certification required at time of start date.
• Independent family practice experience including routine wellness care, chronic care management, and urgent visit, preferred.

Responsibilities

• Provide relationship-based care with prevention, chronic disease management and health education.
• Practice at the top of license with autonomy within Marathon Health’s model.
• Collaborate with teams and leverage resources to improve patient outcomes.
• Deliver wholistic care with an emphasis on prevention and wellness.
